Chicago Public Schools who needs assistance with supplies for their classroom for the Fall 2023 session, we are here to help.

Teachers or principals, please fill out the form below to the best of your knowledge, and we will do our best to provide you with as many of the items as possible. The supply pick up dates are November 4th and 11th (only if needed) from 12pm to 3pm in Des Plaines. The pickup location and times will be provided to you in a private email sent to you after your form is submitted.

Please note that your school needs to be located in Chicago, IL and be considered a Chicago Public School to qualify. Only schools in underserved communities will be awarded supplies at this time.

While we'll strive to provide you with every item on your wish list, sometimes our resources are limited. So please provide us with an accurate quantity for each item so we can make sure we can provide supplies to all the teachers who make requests.

2023 Submissions are now over. Recipients will be notified by September 30th.
